•  An In-home assessment and care plan to meet your family member’s support
   needs.

•  Assistance in identifying and accessing necessary resources.

•  Ongoing observation and evaluation of your loved ones care and support needs

All services are provided by a master’s degree social worker or registered nurse.

Nancy Madrick, RN has been providing care management services for seniors for the past 18 years. She is a member of the National Association of Professional Geriatric Care Managers and completed her care management certification through the National Academy of Certified Care Managers.

In addition to her knowledge of senior health concerns, and her strong assessment skills, she knows the resources necessary to meet the identified needs of her clients. She is ready and willing to advocate for services and support family members in the difficult decisions that need to be made.

Larry Dawes, MSW has been providing care management services since he joined the staff of The Senior Connection in 2001. With a background in management of Alzheimer’s, mental health and rehabilitation, he brings a broad spectrum of knowledge to his care management practice. He demonstrates sensitivity and compassion as he works with clients and their families to make a realistic care plan.

Larry is also a much sought after speaker on topics from Alzheimer’s care and support to memory enhancement activities and safe driving techniques.

He is certified by the National Academy of Certified Care Managers and is a member of the National Association of Professional Geriatric Care Managers, the National Association of Social Workers and the American Society on Aging.
