The goal of Eskaton is to support the independence of older adults in the place they call home. This could be in an Eskaton community or in their own home. The complement of services and care that Eskaton provides – extending beyond the environs of its communities — help reassure older adults that their quality of living will be maintained.
The Senior Connection
These three words mean so much to thousands of Northern Californians each year. The Senior Connection sponsors dozens of educational classes for older adults and caregivers on subjects ranging from fall prevention and dementia diagnosis to advice on better sleep and nutrition. Equally useful, The Senior Connection's professional staff serves as an independent and free liaison for older adults and available community services. The Senior Connection's Caregivers' Resource Guide is an extensive directory of resources in the greater Sacramento area, which also includes definitions of commonly used terms, information on levels of care, and guidance in determining associated costs. The guide is available online, in print and on CD-ROM.
The Senior Connection offers a variety of free workshops throughout Sacramento. Topics cover a wide range of issues important to older adults, including health and wellness, lifestyle, and caregiving. The quarterly Update Newsletter provides a list of all classes.
Ask the Attorney
If you have legal concerns regarding issues related to aging, you can make an appointment for a free, up-to-45-minute consultation with an attorney who specializes in elder law. Attorneys donate their time.
Family and Friends Caregiver Support Group
Support group provides an opportunity for those providing care to family members and loved ones to discuss concerns and problem solve with the help of facilitator, Larry Dawes, MSW, CMC.

Care Management
When family and friends are challenged to make choices regarding the care and support of an older adult, they seek “pathfinders,” such as the geriatric-care managers of The Senior Connection. These experienced professionals assess circumstances, determine resources and develop support plans to help guide individuals through the complexities of caregiving.
TLC Telephone Reassurance Program
Another public service of Eskaton, the TLC program relies on volunteers who contact participants by telephone or home visits on a daily basis. The telephone reassurance and home visits help prevent social isolation, and notify a pre-selected third party if contact cannot be made.
Companionship Plus and Escorted Transportation
The little things can make a big difference. Eskaton provides escorted transportation for medical appointments, as well as shopping, errands and recreational activities. Companions also make social visits to help write letters, chat about current events, reminisce over photographs, even play games.
Home Care Services
Eskaton’s Home Care team of nurses, physical and occupational therapists, and home health aides — through a combination of service, education and encouragement — enables individuals with an illness or injury to remain at home or return there from a stay in the hospital or skilled nursing facility.
Adult Day Health Center
Daytime socialization, therapy, personal assistance and nutrition, provided under medical supervision at Eskaton’s Adult Day Health Center, helps balance life at home for older adults…and caregivers, too. |
