Eskaton Home Support Network provides a comprehensive range of programs and services for older adults who are aging-in-place. “Your home. Our experience.” describes the innovative model that includes the new concierge membership, care management, Telephone Reassurance and Transportation Plus, as well as Eskaton Home Healthcare and adult day healthcare.

Live Well at Home

• Preferred Vendor Program — qualified, reliable business service providers are screened and vetted by Eskaton. These vendors can assist our members from household maintenance to personal fitness, even pet care.
  
  
• Live Well Concierge — a personal consultant to coordinate services, schedule transportation, coordinate group or individual social activities, locate volunteer opportunities and identify personal support services for members when needed.
  
  
• Healthful Living Program — members have access to free and discounted healthy aging classes, organized social and cultural outings, seasonal gatherings, mixers and more. In addition, members have access to Eskaton’s Care Management program as well as our Eskaton Telephone Reassurance.
  
  
• Transportation Plus — members have access to discounted Transportation Plus rides.
  
  
• Home Safety Program — twice-annual preventative service includes replacement of hard-to-reach light bulbs, smoke detectors batteries, furnace filters, and more.
  
  
• Exclusive Online Community — an internet portal allows members to access and schedule services, communicate with staff and stay involved in Live Well at Home social and educational opportunities.

Telephone Reassurance
A public service of Eskaton, Telephone Reassurance utilizes specially-strained volunteers who contact participants by telephone or through home visits. This personal, daily contact helps prevent social isolation, and notify a pre-selected third party if contact cannot be made.

Transporation Plus
Our personalized transportation services provides afforable rides to the grocery store, medical appointments, the airport and personal errands.

Care Management
When family and friends are challenged to make choices regarding the care and support of an older adult, they seek “pathfinders,” such as the geriatric-care managers of Live Well at Home. These experienced professionals assess circumstances, determine resources and develop support plans to help guide individuals through the complexities of caregiving.

Home Healthcare
Eskaton’s Home Healthcare team of nurses, physical and occupational therapists, and home health aides — through a combination of clinical and rehabilitation services, education and encouragement — enables individuals with an illness or injury to remain at home or return home from a stay in the hospital or a skilled nursing community.

Adult Day Healthcare
Daytime socialization, therapy, personal assistance and nutrition, provided under medical supervision at Eskaton’s adult day healthcare, helps balance life at home for older adults . . . and caregivers, too.
