Telephone Reassurance
A public service of Eskaton, Telephone Reassurance utilizes specially-trained volunteers who contact participants by telephone or through home visits. This personal, daily contact helps prevent social isolation, and notify a pre-selected third party if contact cannot be made.
Information and Assistance
With one phone call, older adults, their family members and friends can
receive free information and assistance from a knowledgeable agingservices
professional. Assistance and resources respond to healthcare,
housing, social and financial questions.
Care Management
When family and friends are challenged to make choices regarding the care and support of an older adult, they seek “pathfinders,” such as the geriatric-care managers of Live Well at Home. These experienced professionals assess circumstances, determine resources and develop support plans to help guide individuals through the complexities of caregiving.
Home Healthcare
Eskaton’s Home Healthcare team of nurses, physical and occupational therapists, and home health aides — through a combination of clinical and rehabilitation services, education and encouragement — enables individuals with an illness or injury to remain at home or return home from a stay in the hospital or a skilled nursing community.
Adult Day Healthcare
Daytime socialization, therapy, personal assistance and nutrition, provided under medical supervision at Eskaton’s adult day healthcare, helps balance life at home for older adults . . . and caregivers, too.
