Information and Assistance is a free service available to older adults, family members and caregivers seeking information and assistance on topics that include senior healthcare, housing, social and financial issues, and transportation options. A professional Resource Coordinator can help navigate community resource options.

Live Well at Home by Eskaton provides one-on-one home care support for older adults or people with disabilities in their homes or in independent or assisted living, skilled nursing or hospitals, offering the necessary assistance to maintain a safe and independent lifestyle. Our services are tailored to the needs and interests of our individual clients and we begin all services with an in-depth FREE care assessment. We are available 24/7.

On any journey, a personal navigator comes in handy. Our Care managers – licensed nurses and social workers - are experts in working with individuals, and their families, to identify their goals and locate the specific support services that enhance well-being. When faced with the array of choices and challenging decisions, care managers provide support to find the best solutions.

Services provided

  • Consultation on issues and concerns by phone or in person

  • Evaluation of cognitive and emotional well-being

  • In-home medication management

  • Functional status and home safety assessments

  • Medical appointment advocacy

  • Family communication mediation

  • Ongoing care coordination and updates to distant family

  • Coordination of financial concerns with attorneys, fiduciaries and conservators

  • Legal document evaluation

  • Social supports

  • We provide 24-hour coverage, offering support and recommendations when you need it

Our care managers can assist with the transition to an alternative residence, whether it is an assisted living facility, a skilled nursing facility, or any other residence. Individuals may receive our care management services no matter where they live.

In addition to doing the legwork to find the most appropriate services for their loved ones, we work with family caregivers to navigate the healthcare maze, find respite services, support groups, educational programs, and counseling

Eskaton’s caring volunteers check on the well-being of more than 500 isolated, homebound older adults with daily or weekly telephone calls, providing a free, friendly social connection for people who live on their own and peace of mind for those who care about them.

Escorted rides to medical appointments and grocery shopping are provided to low income Telephone Reassurance participants and Manor residents. Funding is provided by Eskaton Foundation.

Companionship is a good partner with medicine when it comes to health. Eskaton Adult Day Health Center Carmichael -- with nursing, personal care, therapy, social services and activities -- provides a nurturing atmosphere conducive to wellness. Caregivers get a needed break, knowing their loved ones are safe and happy. The program, under the support of California’s Community Based Adult Services, is available five days per week and covered by Medi-Cal where eligible.

Regain and maintain your health in the comfort of your own home through the help of skilled, caring assistance with Eskaton Home Healthcare, a Medicare-certified agency. The team of nurses, physical and occupational therapists, and home health aides help individuals with an illness or injury remain at home or return there from a stay in the hospital or skilled nursing community.
