Coordinator can help navigate community resource
options. Beginning the summer of 2014, Resource
Coordinators will be available 24 hours a day, seven days

Healthy living classes
Social activities and cultural events
Care Coordinators to arrange services, transportation, and recommend healthcare services and preferred vendors
Friendly daily or weekly well-check calls from Telephone Reassurance
On any journey, a personal navigator comes in handy. Care advisors -- licensed nurses and social workers -- will help you design a plan for optimum comfort, safety, and wellbeing.

Eskaton’s caring volunteers check on the well-being of more than 500 isolated, homebound older adults with daily or weekly telephone calls, providing a free, friendly social connection for people who live on their own and peace of mind for those who care about them.

Companionship is a good partner with medicine when it comes to health. Eskaton Adult Day Health Center Carmichael -- with nursing, personal care, therapy, social services and activities -- provides a nurturing atmosphere conducive to wellness. Caregivers get a needed break, knowing their loved ones are safe and happy. The program, under the support of California’s Community Based Adult Services, is available five days per week and covered by Medi-Cal where eligible.

Regain and maintain your health in the comfort of your own home through the help of skilled, caring assistance with Eskaton Home Healthcare, a Medicare-certified agency. The team of nurses, physical and occupational therapists, and home health aides help individuals with an illness or injury remain at home or return there from a stay in the hospital or skilled nursing community.
