Selecting Housing or Care

To assist you in determining the most appropriate level of service, click the box next to the description that pertains to you or your loved one.

AT HOME
Would like to attend a daytime health/social program.
Needs short-or long-term health, and/or rehabilitative services in the home.
Needs a list of home care agencies.
Companionship Plus Needs a driver for appointments, errands and shopping.
Needs daily telephone calls or a friendly visitor.
Needs evaluation by care coordinator and recommendation of services and support.
Caregiver needs time off (respite care).

SENIOR LIVING COMMUNITIES

Prefers independent senior apartment living with exterior maintenance and professional management, but no meals or housekeeping provided.
Prefers independent senior apartment living with meals, housekeeping, activities and transportation provided.
Prefers a retirement community with several service levels, plus the conveniences of meals, activities and transportation provided.
Prefers a continuing care retirement community, which offers several service levels, the convenience of meals, recreation and transportation provided, plus the security of a lifetime membership.
Needs assistance with activities of daily living (bathing, dressing, eating) or medication reminders and prefers the privacy of an apartment or private room & bath and the peace-of-mind of 24-hour staffing.
Requires 24-hour medical attention or has long-term illness.
Needs therapeutic or rehabilitative services (speech therapy, occupational therapy, physical therapy, respiratory care, etc.)
Exhibits chronic confusion or forgetfulness, has difficulty recognizing others, is agitated or has a tendency to wander.
