Eskaton offers a wide range of home and community services.

Adult Day Health Care:   Assists participants and caregivers by supporting the independence and socialization of participants during the daytime hours.

Companionship and Transportation:  Personalized transportation service for appointments, meetings, shopping recreation and social functions.

 
 



Caregiver's Resource Guide: The Senior Connection maintains an extensive directory of resources in the greater Sacramento area. The directory includes definitions of commonly-used terms, information on levels of care and guidance in determining associated costs.
 
 

Home Health Care:  A Medicare-certified agency providing skilled nursing and therapy in the comfort of your home.

Respite and Vacation Care:  Respite and vacation care in a variety of settings to give caregivers a break ...for a few hours, for a weekend or a week.

Selecting Housing or Care:  Assessment in determining the most appropriate service level for you or your loved one.

Senior Care Management:  Assistance in assessing the needs an older adult, identifying resources to meet care needs, implementing services and monitoring ongoing care.

Senior Moving Coordination:  Assistance in arranging and implementing all aspects of a household move by professionals who are sensitive to the unique needs of seniors.

The Senior Connection:  Free information, assistance and consultation regarding a multitude of services to meet the needs of seniors.

TLC Telephone Reassurance Program:  Free daily telephone calls and periodic home visits to seniors who live alone in the greater Sacramento, California, area.
